    What is an e-health record?

    The e-health record (eZK) is a digital version of your medical records. It contains records created by doctors and healthcare professionals — from diagnoses and prescribed medications to vaccinations.

    You log in to your e-health record securely using your ID card with an electronic chip (eID) or electronic residence document (eDoPP) via the National Health Portal.

    What you will find in the e-health record

    Your e-health record is clearly divided into folders to make it easy for you to navigate.

    Person

    Patient identification data — basic information about who the e-health record belongs to.

    Patient summary

    Life-saving data: allergies, implants, chronic diseases, doctor contacts, and administered medications.

    Vaccinations

    Complete overview of vaccinations performed, including dates and types of vaccines.

    Examinations

    Examination descriptions, established diagnoses, health complaints, and proposed treatment procedures.

    Treatment

    Data on the patient's current and previous treatment.

    Prescriptions

    List of prescription and over-the-counter medications that the patient is taking.

    Detailed information

    The patient summary is the most important part of your e-health record. It contains:

    • data on implanted medical devices, including dental implants,
    • information about allergies,
    • contact details of your general practitioner and another attending physician of your choice,
    • diagnoses important for life-saving purposes (according to the Ministry of Health of the SR decree),
    • overview of administered and dispensed medications,
    • contact details of the person designated by the patient,
    • the expected date of delivery.

    Log in to your e-health record

    Access your medical records via the National Health Portal. You need an ID card with an electronic chip and a security protection code (BOK).
